D4 Medical Form

When applying for an LGV or PCV licence, you must conform to a set of medical standards.  Phone your doctor and book a medical examination telling the doctor or receptionist what the examination is for.  Take a D4 form with you to the examination.  You must complete the applicant details and declaration (section 8 on the form) in the presence of the doctor.  The doctor must complete all other sections.  The D4 form also covers the medical practitioners details and must be received by the DVLA within 4 months of the date of your doctor’s signature.  LGV and PCV medicals are not available under the National Health.  You may be charged as much as £90 or more for this service.  Ask your doctor how much they charge.
